There is an example Model Release Form (MRF) as a download on this site. It covers most things, and I am sure you can customise it.

Alamy have a form too, carries their logo etc, but gives you the idea of what you need to include. another here

Quite a funny video, im rather more modest than that bloke. Look at him struting about with that big zoom, i intend to use my equipment to better effect. I got sent a couple of photos from one of the models. They are the last 2 uploaded in my portfolio, she is spanish and looks quite nice. james
