Need help figuring out was is wrong with my technique or gear - flash photos

I want to thank all of you who took the time to critique the image and offer suggestions. You all have been a great help. I have written down all the comments and am sorting them out. Feel free to keep the comments coming.

Thanks everybody for your input. I REALLY appreciate it. Here is what I have boiled it down to based on all the suggestions:

Speedlite Guidelines

Shooting a group - two rows of people

 Select Aperture F stop - F4
 Select ISO - 400
 Meter the scene
o Use P mode and set the F stop to 4, ISO to 400 and note the shutter speed
o More accurately use a light meter and determine the shutter speed
 Switch to manual mode using F4, ISO 400, and shutter speed as determined by above
 Turn on the flash and set it to minus 3 either by FEC or turning the dial on the Speedlite
 Take a shot
o Adjust flash for desired light
o May have to use FEL Flash Exposure Lock so camera is not fooled by a white or dark object
o Adjust shutter speed for desired background lighting
 Note - use a tripod if possible - and especially if the shutter speed is less that 1/60.

What do you think. I have tried it just shooting around the house and it seems to work
