It's a bit too raw for my liking but you get my vote for the emotions this stirs in you when you look at it. On the message front, it works for me, makes me feel like we have to help out more when confronted with such a situation.
On the technical side though, I would have preferred a slightly higher angle to better show the steps of the stairs and better present the person's uphill "struggle", a bit more control over the background (looks too close to being over-expsosed to me), and a slight clockwise rotation to fix the verticals/horizontals in the image.
Regardless, I think this deserves my vote! 
Powerful indeed. The head-on composition conveys the struggle for me - against gravity, against the world.
Oh and belatedly, welcome to one of the most interesting new portfolio here. I see that I have picked up most of your uploads to date from the Gallery!
