To dpf - yes it is a composite image! We did and try and shoot it "live", but the kids weren't going to play ball! On each occasion, the kids were shot slightly off-centre frame with a parent grabbing the kids attention. The chosen shots were very easily merged together with Photoshop.
The lighting is a very easy set-up (I'm too lazy to do complexed ones!), 1x Bowens flash with softbox above the camera, and another head fired at a white wall. Its that simple!

I would be VERY interested in a quick sketch (via Paint or similar)of your light setup if you had a spare second. I am at the start up stage and just not confident with lights yet.

The lighting set-up is dead simple....
I'm lying on the floor with my camera aimed at the subject. Above me is the key light - 1x flash head with an approx 40" softbox aimed at the subject. The other flash head has a medium sized dish on it which is aimed at a white wall positioned behind the kid(s). The exposure of the background light is approximately 1 stop more than the key light to ensure that the white background is white and not an off-white/grey.
That it all there is to it.
