There are two ways of doing this, naturally, or in PS.

The latter will involve selecting the arms and club, placing them onto a new layer and moving the rotation point. Copying the layer and using a mask for each one will allow you to blend the stages of swing. Of course, the disadvantage of doing this is illustrated in your image, which shows that elements such as the watch will be in view at one point, but not at the lower end of the swing. However, it will allow you to add a slight amount of motion blur without worrying about the background. I think you will need to depict some movement.

Going this naturally will be more accurate. Using a tripod, firmly locked, ask the golfer to stand in exactly the same position. Take loads of images using manual exposure / focus and select those which show the swing at equal intervals. Because the background will be static, you can the use a hide all layer mask (black) on each shot and then blend in using a white brush. Adding blur will be more difficult because you'll blur the background as well.

To get the best of both techniques, you'll need to use the natural technique and select the arms and club, as for the PS only technique.

Lovely depth and great eye contact. My only suggestion would be to darken the upper right area, which tends to drag the viewers eye away from the subject. Has my vote.

Beautiful shot and model, but it's such a shame that the reflection is so inaccurate. The only way to achieve this is to place elements of the model on different layers and transform each one individually. The reflection of the heel should meet the touchpoint on the floor and merely transforming the hand vertically will not work because you won't see the top of the hand, only reflections of the fingertips. Apologies for being harsh, but if i've spotted this then others will too and I hope what I have said will help you perfect future images.
