I noticed the nipple decoration and an idea sprung into life. I can see an image here of a shot of this models right breast as the subject with the nipple decoration being primary focus and a shallow depth of field blurry the rest of the breast and the left breast. I imagine either candle light or honeycomb to create the right light. Then just add a little diffuse glow in photoshop and bob's your uncle! I reckon that would work reallt well.
