Not quite sure why you've marked this image as "Adult" - I'll try to get it removed for you.

The lighting on the boy is very nice - it's even but is not flat, so you can see that his nose sticks out from his face etc.

In terms of composition, it's a bit bizarre. The landscape orientation for a portrait would normally only be used if you were trying to show the surroundings; the busy background here is not very suitable for this technique.

I would try to use less DOF to knock the BG out of focus - this could be done in Photoshop or your favourite processing program, though.
