Canon eos 3 + Canon speedlight 540ez + 500mm lens

It should be fine, within reason.
I have the above combination myself though never used them together but have used flash with a telephoto.
The 540EZ will automatically narrow the beam albeit to that required for a 105 mm lens field of view. The only issue id making sure the subjects aren't too far away, though a full length portrait at 599 mm is pushing it. Even with a more concentrated beam it's surprising how the light intensity is less than you'd like. However, as you only want it for fill-in rather than full illumination that should help. I guess the flash will be pumping out at or around full power in any case so rapid shooting won't be an option.
It may be obvious, but an ISO 400 film would be of benefit too, as would using the lens at maximum aperture. If it were digital I'd push the ISO to 1600 or even 3200.

You would be better off mounting the flash on a stand positioning it a few feet from your subjects and using a longish flash lead or wireless trigger with longer lenses.
