Really cure child, with a great expression! For me the problem with the background isn't so much the clutter - this gives a natural feel to the shot - but the two highlights that draw the eye. Also, I can see most, but not all of the child, The framing and composition makes me miss their feet.
The real image here is the childs smile, and I'd do a fairly major crop of her head and shoulders to produce another image.
