Thanks for the comments Paul.
First off, the photo was taken in full glorious colour, I'd opened the curtains and the sun flooded in!!
I then used a set of free PS plugin filters from http://www.optikvervelabs.com/, some really nice presets, but with the option to fine tune to your liking.
Finally ran the finished shot through NeatImage another free software download for reducing noise not that there was much in the shot anyway but it gave the final image a smooth porcelain look that I really find pleasing.

Quite like this but I do have reservations in that I find it a touch too sharp and the skin too smooth. Could be that you have over Neatimaged it slightly. I would just apply a very slight amount of gaussian blur to knock the edge off it and rotate it 90 degrees CW. Lighting is good. That optikverve plugin is an excellent piece of kit.

I can see where you are coming from with regards the "too sharp-too smooth" comment.
The rotate 90degs is something I'd not considered (currently sat as though my neck is broke lol) I'll have to have a play, if I change anything then I'll repost
